A rapidly growing regional healthcare network needed a single mobile platform to bridge the gap between its clinical teams and a patient population spread across urban and rural areas, many of whom had limited access to in-person care.

Our client operated 14 clinics across three states. Their patients, many managing chronic conditions like diabetes, hypertension, and heart disease, had no reliable digital touchpoint between visits. Follow-up calls were missed. Medication schedules were forgotten. Remote patients drove hours for 15-minute consultations.
