The goal was to build a centralized platform that brings together glucose monitoring, medication management, nutrition tracking, activity monitoring, patient engagement, clinical decision support, and remote care workflows.
By replacing fragmented diabetes management tools and manual tracking processes with an intelligent cloud-based platform, the client improved patient engagement, streamlined care coordination, reduced administrative workload, and enabled healthcare providers to gain real-time visibility into patient health trends and diabetes management.
People living with diabetes often manage glucose levels, medications, nutrition, exercise, appointments, and health records across multiple disconnected tools. Healthcare providers may also rely on manually reported glucose readings and periodic consultations, making it difficult to identify changing health patterns and intervene proactively.
