Project Overview

CareSync is a custom remote patient monitoring platform developed for a chronic care management provider seeking to improve patient visibility, reduce administrative workload, and scale monitoring operations. The platform unified wearable device data and clinical records into a single system, helping care teams identify patient risks earlier while supporting long-term growth.

Client Background

The client operates a chronic care management practice in the US, working with patients who need regular monitoring between in-person visits. Before CareSync, their team tracked patient data across separate apps and spreadsheets, with no single view of a patient’s condition over time.

They came to us looking for a platform built around their own patient intake process and care team structure, not a packaged tool they would have to adapt to.
